The couple have two daughters together, one born in 2009 and the other in 2015. The family lives in Pacific Palisades, California.
When Smulders was 25, she was diagnosed with ovarian cancer while filming How I Met Your Mother. She underwent two surgeries to remove two ovarian tumors, but by then the cancer had already spread. She then had to undergo a number of other surgeries over the next two years to continue fighting the cancer. She announced in 2019 that she is in remission.
